Exceptional Tree and Palm Nomination

Step 1.Take a photo of the tree(s)/palm(s)

You will upload this digital photo to the form linked below in step 4.

Step 2.If the tree/palm is located on a CPR, create a site map and distribute nomination forms

• Create a site map that includes and indicates all other CPR units and the respective owners.
• Distribute the Notice of Nomination of an Exceptional Tree Form(PDF, 201KB) to each CPR unit owner.
• You will upload these files to the form linked in step 4.

Step 3.Arborist report on health of tree(s) or palm(s)

This is recommended and will be uploaded to the form linked below.

Step 4.Complete the application form online

Complete and submit the Exceptional Tree and Palm Nomination Application Form.

Step 1.Print a current photo of the tree(s)/palm(s)

Take a photo of the tree(s)/palm(s) and print the photo(s).

Step 2.If the tree/palm is located on a CPR, create a site map and distribute nomination forms

• Create a site map that includes and indicates all other CPR units and the respective owners.
• Distribute the Notice of Nomination of an Exceptional Tree Form(PDF, 201KB) to each CPR unit owner.

Step 3.Arborist report on health of tree(s) or palm(s)

Step 4.Download the PDF form

Step 5.Open the PDF form

Open the PDF file on your computer and type your answers into the fields.

Step 6.Print your completed form

Print your completed PDF form.

Step 7.Return form(s) and attachments by mail
