An interactive 6-week workshop on self-management skills for chronic health conditions, diabetes, or cancer survivors.

In-home assessment that includes a short home visit by Agency on Elderly Affairs, Kauai Fire Department and American Medical response.
Family caregivers (grandparents) have supportive programs and services that address their needs to enable them to continue giving care.
Helping frail older adults with housework, meal preparation and other limited in-home services. To qualify, someone must not be able to do one or more activities of daily living (ADLs) or instrumental activities of daily living (IADLs).
Referral of in-home workers who provide services for a fee.
Provide adult day care services in a supervised, protective, and congregate setting during the day.
Bus service available for registered seniors to and from program activities, shopping, clinic, recreation, and door-to-door services for frail elderly.
Legal education, counseling and referral in Medicaid, Medicare, landlord-tenant, creditor, family law and other legal matters. Limited document preparation.
Hot lunches at various congregate meal sites and island-wide home-delivered meals to homebound seniors.
To qualify for a home delivered meal, an individual must be 60+ years old who is homebound and frail with two or more impairments and an unmet need, and meet at least one of the following:
Screening criteria will be provided to determine eligibility. Voluntary donation is accepted.
Provides caregiver relief by assisting in the supervision and care of frail elderly.
Meaningful volunteer opportunities for persons 55 years and older.
One-on-one Medicare counseling.
Senior identification cards are available to seniors 60 years and older. IDs are issued twice a month. Appointments are recommended by calling 241-4470. A consumer registration form must be completed. A donation for the cost of the materials used is accepted.
